Service Description

Have flu forms completely filled out prior to appointment. These forms can be found on our website for you to print at home. We are unable to print forms in our office but we will have child and parent forms available if needed. If you are a new patient to our practice and have not been seen by your physician yet, please call the office to schedule your shot. We are offering parents flu shots as well for a $35 fee (we will NOT file with your insurance We have FAQ’s regarding the flu vaccine itself on the website as well, but if you have further questions please call 881-0007 and follow the prompts for the Flu Clinic Voicemail box. This box will be checked biweekly, so if you have a time sensitive question please contact your doctor’s nurse directly. We look forward to seeing you soon!
